Procedure
Fill the form
Landline or Mobile no. is mandatory
Website / Skype / Face book is not mandatory
Any pen , no color specification
but it should be visible
Both the sides of the form should be filled
After
filling the form you have to
take the pic of the candidate who has given details.
After filling
the form kindly hand over the
lottery ticket to the candidate
Now you have to scan both the sides of the
form and send it us along with the photo
and with the serial number mentioned in
form.
Rejections
q if any one found charging even a single
penny from the general public for filling these forms by the name of the gifts

any one uses duplicate forms

the forms provided by the company are acceptable

contact number is missing

Payout
Per form Rs. 14/-
Processing Charges
q There will be only one state coordinator
under which districts will work.
q District-Rs. 2,00,000/-, qty of forms 50000
q State coordinator will get authority from the
direct company whereas district coordinator will get authority from the state
coordinator
q Although
there is no rejection criteria but in case of the following the agreement will be terminated the same moment with no notice
Submission
q Submission of forms on every 1st of every month
q Payment on every 20th of every
month
